Eyeworks Policies

Payment for services are due when rendered. Sorry, we do not bill you later for services rendered. If you can not pay for your services at time of exam, we will gladly reschedule you at a more convenient time. If there is a circumstance where your insurance company denies our claim, we will bill you at that time.

All Sales Are Final/No Refunds. Eyeglasses and Contact Lenses are not only medical devices but custom. Once you place your order, lens design will begin and frame/and or contact lens order placed.

We understand that you do not always know who your vision insurance is (or if you even have coverage). We will make every effort to find your plan. However, if you we can not and you do not know who your insurance is with or if you even have insurance, you will be billed our usual and customary charges. We will provide you with an itemized receipt to submit if, at a later date, you determine you had coverage. Eyeworks does not have the capability to bill after services are rendered and materials are ordered.

Optician Measurements (PD, OC height, etc) are NOT part of your Eye Exam. These specialized measurements are taken by the Optician where you are ordering your glasses and work in conjunction with the frame and lens of choice.

Contact Lens Exams are separate exams which can be performed in conjunction with your Eye Exam. There is a separate charge for these exams. Eyeworks offers policies and charges along with patient paperwork. We also charge in alignment with your insurance company dictation. If you call your insurance company for clarification and do not tell them you are in need of a contact lens exam AS WELL as an eye exam, they will not tell you about an additional charge. Contact lens exam charges will vary depending on the type of contact lens exam you require.

Progressive Lenses have a non-adapt policy. If you decide you do not want progressive lenses after order or dispense, we will gladly switch out for bi-focal or single vision within 2 months of purchase.

Eyeworks is not responsible for incorrect prescriptions written by an outside doctor. We do stand behind our own. If there is uncertainty about your outside prescription, speak to one of our eye care associates about seeing one of our highly skilled eye doctors.

Eyeworks has a strict 24 hour cancellation policy. Please contact us within that time if you need to cancel or reschedule your exam appointment to avoid our CANCELLATION FEE.
